Need your help for tax purpose

Hi, during the year 2013, my son went to familiar garderie for full year of 7$ per day. But i was moving out of the city on febuary and i forget to take the SHEET to do the tax ( i dot know exactly but something like relieve 1 or t4... the paper that i need all information in column A,B,C,D to do the tax. Now i live every far and i can't call the educatrice. Anyone here have that information, please provide it to me so i can do the tax. I know i won't get any money from government because it is already 7$ but i still want to do the tax clear. Thank for your help:rolleyes::rolleyes:
